Material Location
Provide the URL or other specific location where the alleged infringing material is located.
Contact Information
Include your contact information, including address, telephone number, and email.
Good Faith Statement
Include a statement that you have a good faith belief that the use is not authorized by the copyright owner.

Counter-Notification
If you believe your material was removed by mistake or misidentification, you can file a counter-notification. The counter-notice must include:
- Your physical or electronic signature
- Identification of the material removed
- A statement under penalty of perjury that you have a good faith belief the material was removed by mistake
- Your name, address, and phone number
- A statement that you consent to local federal court jurisdiction
Note: Making false claims or misrepresentations may result in liability for damages under Section 512(f) of the DMCA.
